What the hell is the issue ? please.
Last command before the server shut down is "/Stopanim" but I don't think that's related .
Maybe the samp03 folder needs 777 chmod ?

Do you have an idea of when it happens? Also it seems like mysql may be the issue. Is there any mysql function you added recently that may cause it?

A public production environment (or anything public) should never have 777 permissions unless if it's something like cache files, upload directory etc.
777 equals: rwxrwxrwx, which is in the format of rwx x2 (user-group-other). r = read, w = write, x = execute.
This means that:
user can read, write, execute
group can read, write, execute
other (eg. public) can read, write, execute

777 is binary format: rwx could be seen as 421. So rwx is 7 (4 + 2 + 1), r-- is 4 (4 + 0 + 0), r-x is 5 (4 + 0 + 1).
Usually for files that need to be executable for non-owners (like public) you want the permissions to be either r-- or r-x (4 or 5). You do probably want the user and group to have full access over it so that would be 775 (rwxrwxr-x).

You may have noticed with command ls (-la) that you see those very characters in front of files/directories (this is just an example):
Code:
drwxrwx--- root root .
drwxrwxr-- root root ..
dr--wrx--- kwarde dev logs
-rwxrwxr--x kwarde dev somefile
First character is either 'd' or '-': d means it's a directory, - means it is a file. Then you see the rwxrwxrwx line (- means they don't have that permission). Then you see two names and then the actual file. The first name is the username that is the owner of the file, and then the group that is owner of the file. After that you see the actual directory or file

You may have known this already but since you suggested to set it to 777 I'd figure you didn't. If you already did, disregard this message.

The mysql.so plugin had an exception.
Did you change anything related to it? Double check that you have installed the required stuff for mysql to work.

To be honest it happens sometimes suddenly.. and it started long time ago so I don't remember what I added or changed is there any way to trace it ?

If you don't mind having a "flooded" server log, add a print to every single callback, and see where this goes.
